It can be due to a medical reason - such as urinary tract issues - and medical causes should be ruled in or out at the beginning of the work up. However, many litterbox behaviors are linked to anxiety of some sort. This can be due to a myriad of factors, including interaction with other pets (especially other cats) or people in the home, litterbox issues (location, number, litter material), or other causes.
